The Hounslow GP Consortium CIC and its voluntary sector partner, Groundwork London, have a new post available for committed health and care staff to work across Hounslow’s Primary Care Networks as

Social Prescribing Team for Chiswick PCN

  • Our Team Leaders manage the development and delivery of the social prescribing programme supporting residents within their PCN
  • Provide connections between the GP and wider community services, to address their patient needs in a holistic way
  • Oversee and supervise a team of Link Workers supporting the team to provide a quality service and achieve their annual caseloads targets

We are seeking individuals with a background or interest in health, care, public health or voluntary sector services. Experience of managing and leading teams and projects. Experience of social prescribing is desirable but not essential.

The Link Worker programme is a national initiative from ?NHS England and NHS Improvement. These posts are employed by Groundwork London and seconded to Hounslow PCNs. Please note this post is subject to an Enhanced Criminal Records Check suitability through the Disclosure and Barring Service.
